Stricter monitoring of interments/funeral service, inventories, revenues 

Objectives:

     1.  To ensure proper matching of revenues and collection;
     2.  To ensure that the revenues due the company are all accounted for;
     3.  To thwart theft and misconduct, wherein an sbu is a suspect

  Reality:

   1.  There are reports in region 1 of the  following:

      1.  Unreported interments
      2.  Missing marble tablets
      3.  Sale of used markers

   2.  The perpetrators are in the highest rank of sbu, and instead of stopping
        old practices and thieving

  PROCEDURES WHAT TO DO:

  1.  Making of vaults:

     1.  Pictures of receipt of materials  picture of DR

     2.  Pictures of payment and OR by the supplier

     3.  Picture of the finished vault with numbers and arranged in sequence

     4.  Picture of the contractor:

          1.  Receiving payment

         2.  Issuing his AR/OR

        Due;   following date after the completion

  2.  Usage of vault interment:( upon notice of death)

j     1   Upon filling up of IA, the sbu sends picture of IA

      2.  OR with the cash (close up) of payment made   and the Authorized representative

      3.  Deposit slip of the payment made

    prior to making of cash advance for iF

      4.  Picture of the vault to be used. with the number visible

      5.  Notification by the SBU of the interment:

           Number of interment for the month
            Cumulative number for the year

           1.  Name      Age            COD   Address
           2.  Death date                  Interment date
           3.  Type of interment
           4.  Price                            Amount paid
           5.  Vault number used
           6.  CSS
          7.  Date of Obit                 Where             Link

              Due date:    the date of IA.   

             to be sent to Area Head  third floor.   The AH shall compile this monthly and submit not later than 5 days after end  of the month

       Picture of the interment:    the procession and the interment process

3.  Markers (New and Used)

    1.   Picture of all marble blanks being delivered

    2.  Picture of receipt of payment

    3.  Numbering of blanks with pen tel

    4. Picture of  storage of marble.

    5. Inventory list of the markers  (new)

        Picture of withdrawal

       Due:    date after withdrawal is made

  6.  Inventory of used markers

     Due :  the next date after withdrawal is made

PENAL PROVISIONS:

 1. Any missing report/picture shall get a demerit of P100.00

 2. Non submission or delayed submission is P500.00 penalty

3.  Dismissal with criminal prosecution for habitual violation

4.  Those who have resigned previously, after due investigation and research shall be prosecuted to the full extent of the law, and sent to jail
     even with settlement

N.B.

1.  No amount of process can stop determined thieves and wily thieves

2.  It is "sayang"na talents that  instead of righting the wrong, some new recruits made things worse.

3.  The misconduct committed does not speak well of the residents, especially that if there are allegations that those ahead did these

4.  Lesson learned:   be careful of new recruits, do BI question their statements and representation

5.  Late reports and submission, insubordination of our procedures can only mean one thing.   

6.   Beware of deception, diversionary tactics antics and emotional outburst when a personnel is questioned.
