Reminder for the standards of PPA and ledgering

1.  Filling up the form


It   is printed in quadruplicate and is done in one page.  All the required information are in the first page, including the data required for GCL.  The PPA should contain the correct and clear signature of the customer.   The back page contains the Terms and Conditions


As much as possible, the blanks/boxes are to be filled up by the customers or their relatives, or dictated to our staff.


      The important data are:


     1. Account number

     2.  Printed name of the customer

     3.  Address (printed)

     4. Contact number cp

     5.  Contact email address

     6.  Plan name    amount    initial payment

     7.  Term:  annual, semi annual, quarterly monthly

     8.  Amount of amortization


   The following should be observed strictly:


     1.  Print the data clearly and legibly

     2.  No blanks are to be left;

     3.  SOCPN (signature over complete printed name) for all signatures:


              the customer

              the seller

              the officer/employee who approved the ppa


   4. Insurable/not insurable (please check).  Those who are not insurable are:


      above age of 59 and below 18

      are engaged in risky jobs:   miners, seafarers, police

      previous medical conditions


   5.  a photocopy of a valid govt issued id shall be required from the customer/buyer (front and back)


2.  For ledgering and accounting purposes, the data are in the PPA.  The account number is at the upper left hand corner.  We will use it for numbering the ledger and the reference number for the customer.  We provide ID card or slip to the customer


    The data required for the ledgering are:


    1  Account #

    2   Name of customer

    3.  Address

    4.  Contact # cp

    5  TCP

    6.  Term

    7.  Periodic payments

    8.  Name of agent

They are obtained from the PPA


The same could be obtained from the abstract/portal


3.  Once the payment has been received, and the form filled, and signed, the branch officer/staff can initial and sign the PPA.   The yellow copy is given to the customer


4.  The 3 remaining copies are sent to the HO for approval and confirmation by a Senior officer..  The original is sent back to the branch for the customer, another copy is retained by HO for file and another copy stays with the branch:


        Original     -  customer

        Yellow       -   customer

        copy         -    branch

        copy         -    records HO
