That I have been authorized by the family of the deceased to speak for and behalf of the family and all concerns shall be addressed
through me...No other parties will direct nor alter
That only communication in writing through me shall be entertained by the company
That only the service features mentioned above shall be provided. Additional and extras are subject to further requests and pricing
2. That the deceased has no medical legal cases; and if ever there is; the customer shall hold the company free and harmless from claims or lawsuits
3. That the deceased did not die from hazardous materials nor communicable diseases which requires different modes of burial;. In such case, the
customer and the family shall compensate the company or those who were affected by the communicable disease
4. That the deceased does not have any device nor gadget, as in the case of cremation that may cause hazards in the atmosphere nor cause
explosion or fire for the cremation machine in which case the company shall be compensated without need for demand
5 That the authorized representative wrote/dictated the correct information as the name and circumstances about the deceased and any such
errors shall be blamed on the company
6. That to the best of its ability the company has provided for the needs of the family as per standard contract and shall not be held responsible
for anything that is not contained herein or agreed upon
7. That for discounts requested the level of service shall fall accordingly to the price level that has been reached. Service that is provided at full
price can not be provided for packages that have been discounted. The company can not guarantee full satisfaction for discounted prices that
are deep
8. Only representation and features that are in writing and contaiend herein shall be provided by the company and representation on the contrary made by individuals can not be entertained
