Subject: Return of Application for CGLI and PAI coverage of Hataw March buyers
We sent this notice last September 21, or nearly 2 months ago.
The sad state of our affairs is that out of 5,000 or so buyers only about 300 have applied for insurance are covered. The insurance coverage is by no means automatic. Even the 300 are not covered by PAI
This office gently reminds the SBUS and their CSS (2 or 1) to attend to the submission of the above since the non coverage puts HGMP in great risk:
1. Financial risk - in case of death or accident (since that is stated in the contract); we may not have the PHP to pay the claims especially for accident
2. Legal indemnity - because we agreed in the PPA that there is an insurance coverage.
This office enjoins your good selves and it is a fair warning to attend to this immediately. The pending items pile up
This office requires that you submit to this office immediately the timelines and commitment for action

September 21, 2017
TO: All Holy Gardens Buyers
Notice hereby served to all the buyers of Holy Gardens Memorial Park under our regular sales and Hataw Promo last March to May 2017.
We promised that we will issue all our buyers for Personal Accident Insurance and Credit Group Life Insurance which assures the customers of amortization deemed paid and disability benefits in PAI.
However, this shall be effective after you have filed GLI application form an after you receive a policy for such insurance coverage.
Holy Gardens Memorial Park its officers, employees and assigned agents shall not be held liable for such insurance coverage unless a duly signed and accomplished application for Group Life Insurance has been submitted to us and policy for such coverage have been issued.
A large number of our customers unfortunately have not done the above. May we request for usual and kind consideration and understanding.
