SUBJECT:  SETTING UP OF HELP DESK FUND



I  Background:  (Reality)

We are in the bereavement and memorialization business.   However, 70% of those who die are poor and destitute and do not have money to bury their dead.

We are thus entering into the Chapel and Memorial Services to address the issue of :   affordability and death and poverty

B  Objectives

In our mission statement, we mentioned that to address affordability:

1.  We shall have indigent packages of P12t and work closely with office of VM and the LGU who are setting up same assistance to the poorest of the poor.  This low cost service plus contribution for those who sympathize and other NGO, we can help reduce problems of the poor

2.  We also have the P20t at need package to help those who have funeral assistance from SSS and GSIS

3.  We also want to assist the indigent (the poorest of the poor with their burial needs)

      2.  through help desk fund. We have asked the AP to set aside 3% of  GP of interment fee for this purpose but has not been implemented


III   In order to implement this institutionally and order to make ourself count, the following shall be implemented

What :   Setting of Help Desk Fund at Majorem Lending.  Theis fund shall be used to assist the poorest of the poor.

      Maximum donation:   P3,000.00 per deceased

     What:    supporting documents:

         1.  DSWD certification

          2.  Letter from Barangay of OVM

How much:

          5% of GP of:

        1.   IF

        2  Chapel and Memorial Services

        3.  IMP and MP

Who:   Majorem Lending shall be:

1.   Safekeeper of funds

.   Disbursement of funds upon presentation of proper docs mentioned in # 2

Who pa:     CSS and CMO shall inform 3ffb of those who need the funds.
