What:
As per our conversation yesterday, kindly have the following study made on our forms (PPA). There will be revision and the record keeping typing will be decentralized with data capture, and central archiving of copies of documents:
OUr benchmark is SPLP application form and terms and condition, with ID and Certificate of Payment that I showed you.
1. PPA
CRM and health questionnaire in front
PPA and terms and conditions at the back (in say 8 pts) letter
2. ID issuance (continuous form)
3. Final payment:
CO on top (with space for Deed of Assignment)
Deed of Sale at the Bottom
in one page; use of continuous form
You are directed to canvass for IT/consultant who will do this for us.
