Good day! Attached herewith the Control Achievement Sheet per positions. 
This is to determine the efficiency of employees towards work.

Please be informed that all employees must submit it weekly to HR. This will reflect to your Performance Evaluation and it will be the basis for Merit increase.

Failure to submit your accomplished Achievement Control Sheet will dealt to disciplinary action.


Under Code of Conduct:

 Section 2. Offense against Performance of Duty

2.28   Any acts of insubordination or disobedience   -  Class D

2.31  Failure to submit assigned work on agreed time -  Class C
